(1) Physicians include physical activity and other healthy behaviors in reviewing vital signs with patients during medical visits. (2) Physicians refer most patients to the medically-affiliated community health clinic (staffed by credentialed kinesiology students) or to local physical activity and wellness programs, or to the university-based health and fitness assessment program. (3) Patients develop physical activity habits and supporting healthy behaviors at the medically-affiliated community health clinic with the help of kinesiology students. After developing autonomy, patients are directed toward local physical activity and wellness programs offered to the general public. (4) Physical activity and markers of health and fitness can be measured and monitored in the medically- affiliated community health clinic or by the university’s assessment program. (5) Physical activity, health behaviors, and the markers of health and fitness are part of an integrated database that is accessed by physicians and other members of the collaborative team to review with patients and to enhance patients’ health, fitness, and well-being.
